Please fill out and submit the below form in its entirety – for each individual product (even if it shares the same batch number).
Customer has 30 days (from date of submission) to submit all evidence of information or the claim will be denied due to insufficient evidence and information.
‘Assistance’ will follow up with The Distributor once the claim is received and processed.
A 12” x 12” defective sample is required (to be placed on a spare liner). Please ship the sample to HEXIS Americas / Attention: Claims.
